Licensed Medical Social Worker / LMSW (Home Health)

Summary: Mission Healthcare, located in seven states, is the largest home health and hospice company in the western United States. They are seeking a Licensed Medical Social Worker to conduct psychosocial assessments and develop intervention plans to support patients and families in their healthcare journey. Responsibilities: - Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment - Develop and implement tailored intervention plans based on assessment findings - Maintain accurate clinical records for all patients referred to social work services - Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs - Serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services Required Qualifications: - Master's degree in Social Work accredited by the Council on Social Work Education - Active license as a clinical Social Worker by the Board of Behavioral Sciences - If you reside in Oregon or Washington an LCSW or AWS is required - Minimum of 1 year of experience in healthcare, with preference given to candidates with home health care experience - Current BLS, hands-on CPR certification - Valid driver's license and reliable, insured transportation - Excellent verbal and written communication skills, along with strong organizational abilities Required Skills: Master’s degree in Social Work, Active clinical Social Worker license Important Skills: 1 year healthcare experience, BLS CPR certification Nice-to-Have Skills: organizational abilities Benefits: 401(k) retirement savings plan, Mileage reimbursement, Employee Assistance Program (EAP), Paid vacation, sick leave, and holidays, Medical, dental, and vision insurance, Flexible Spending & Health Savings Accounts, Disability, life, and AD&D insurance, Pet insurance
